Objective To explore the safety and efficacy of local laparoscopic resection of organ function retention for benign and low-grade malignant pancreatic tumors.Methods From July 2020 to December 2021, clinical data were retrospectively reviewed for 28 patients undergoing local laparoscopic pancreatic resection. There were pancreatic head mass (n=16), laparoscopic duodenum-preserving pancreatic head resection (n=13), laparoscopic enucleation (n=3), pancreatic neck & body mass (n=7, including laparoscopic central pancreatectomy, n=6; laparoscopic enucleation, n=1) and pancreatic tail mass (n=5, including laparoscopic spleen-preserving distal pancreatectomy, n=3; laparoscopic enucleation, n=2).Results All laparoscopic procedures were successful without any conversion into laparotomy. Operative duration was (60-450) min, intraoperative volume of bleeding loss (20-550) mL and tumor diameter (2-8) cm. Postoperative complications were grade B pancreatic fistula (n=6), grade C pancreatic fistula (laparoscopic enucleation for pancreatic head mass, n=1), abdominal infection (n=2), biliary fistula (n=2), gastroparesis (n=1), abdominal bleeding (n=2, including laparoscopic enucleation for pancreatic head mass, n=1; laparoscopic central pancreatectomy for pancreatic neck & body mass, n=1). During 8-month postoperative follow-ups, there was no instance of secondary pancreatic endocrine dysfunction. The average hospital stay was 20.9 days. Postoperative pathological examination indicated that there were solid pseudopapillary tumor (n=8), pancreatic neuroendocrine tumors in G1 stage (n=5), G2 stage (n=2), G3 stage (n=1), serous cystadenoma (n=5), intraductal papillary mucinous tumors (n=4), chronic pancreatitis (n=1), schwannoma (n=1) and congenital true cyst (n=1).Conclusion Local laparoscopic resection for benign or low-grade malignant pancreatic tumors can effectively preserve organ function. Both safe and effective, it is worthy of wider popularization.
